The Patient’s Story: A Quest for a Perfect Smile

This image highlights severely unstable upper dental bridges, compromised posterior bite, and significant difficulty chewing. An essential reference showing the life-changing potential of advanced digital dentistry and surgical expertise in Toronto.

Before Smile Transformation

The patient came to me seeking not just dental care, but hope. She was in desperate need of someone who could navigate the intricate pathways of surgical dentistry. Her condition was complex and challenging: the anterior upper bridges were so unstable and loose that everyday tasks—simple actions like biting into food—had become nearly impossible. Every attempt at functioning reminded her painfully of her compromised oral health.

"Initial 2D Digital Smile Design by Dr. Johnson Ozgur at Toronto Smile Design" This image showcases the precise 2D digital smile design created by Dr. Johnson Ozgur, illustrating the initial treatment plan for addressing complex bite and aesthetic issues. Using advanced digital planning at Toronto Smile Design—Yorkville Dental, this step provides patients clarity and confidence by previewing potential outcomes before treatment begins.

The Challenge: Beyond Aesthetics

Beyond the instability of her front teeth, the patient faced an equally daunting issue at the back of her mouth. Her posterior bite was severely compromised; indeed, the entirety of her dentition met in just three isolated points, turning even essential chewing into an exhausting and frustrating ordeal.

Before reaching our practice, she had consulted three other dental professionals and specialists, each deepening her uncertainty and distress. She was searching not merely for a treatment but for clarity, assurance, and expertise—someone who could restore both her bite and her confidence. She was looking for a solution that could be offered and finalized under one roof.

The Treatment Plan: Personalized and Comprehensive

Following the detailed 3D printing of her customized designs, we presented her with a mock-up and a comprehensive treatment plan presentation.

This crucial stage allowed her to envision the possible outcomes clearly before committing to the procedure. More importantly, it allowed us to thoroughly inform and educate the patient about essential aspects of her care:

  • Diagnosis: Identify the underlying issues contributing to her current condition.

  • Nature and Purpose of Treatment: Explaining specifically what procedures would be involved and how they would address her concerns.

  • Risks: Transparently outlining potential complications or challenges associated with the procedures.

  • Benefits: Emphasizing the improvements and positive outcomes expected from successful treatment.

  • Prognosis: Giving an informed perspective on the treatment's expected long-term outcome and success.

  • Reasonable Treatment Alternatives: Discuss other options and compare their potential outcomes and implications.

  • Consequences of NOT Having Treatment: Highlighting potential deterioration or ongoing complications if the condition remains untreated.

  • Cost: Communicating the financial investment required for the proposed treatment, allowing for informed decision-making.

This detailed and informative approach ensured the patient could move forward with confidence, clarity, and informed consent, laying a solid foundation for successful outcomes.

Bone Grafting

After gaining the patient's approval, guided by the CBCT imaging she provided, we proceeded with extracting the compromised teeth and grafting the extraction sites with allograft materials and protective membranes. This critical step is paramount for future implant placements as it preserves the bone structure and encourages optimal bone regeneration, creating a stable and robust foundation necessary for successful implant integration.

A fixed temporary solution

Additionally, recognizing the patient's desire for an excellent aesthetic and functional interim solution, we utilized her remaining teeth to provide a fixed temporary bridge during the healing phase. The long-term fixed temporaries offered numerous advantages, including preserving the patient's oral aesthetics, maintaining speech clarity, allowing comfortable chewing function, and providing psychological comfort by enabling her to continue daily activities confidently and naturally throughout the healing process.

Complete MODJAW Dynamic Documentation

Once the implants had fully osseointegrated, we proceeded with MODJAW documentation to meticulously capture the patient's natural jaw movements and relation. This innovative step is critical, as it ensures the prosthodontic design harmonizes seamlessly with the patient's unique physiological function, enhancing comfort, function, and longevity of the restorations. Leveraging advanced digital technology, we precisely designed and manufactured the final restorations. This digital precision not only improved the aesthetic outcomes but also guaranteed accuracy in fit, function, and alignment with the patient's natural bite and articulation, setting a new benchmark for personalized dental care.

The Results: Hybrid Implant Supported Prosthesis

For the final restorations, we implemented a hybrid approach in designing the implant-supported prosthesis. This technique allowed us to replicate the natural soft tissues that had been lost due to bone resorption over the years, greatly enhancing aesthetics and overall functionality. Additionally, we chose screw-retained implant prostheses over cement-retained ones, significantly improving the longevity and ease of maintenance of the implants. Screw retention facilitates simpler repairs, adjustments, or replacements if needed, reduces risks associated with excess cement causing peri-implantitis, and ultimately ensures the long-term stability and health of the patient's dental implants.

Dr.Johnson Ozgur - Cosmetic Dentist - Oral Surgeon - Toronto Smile Design

Comprehensive Smile Transformation: Complex Surgical and Restorative Dentistry at Toronto Smile Design—Yorkville Dental

At Toronto Smile Design—Yorkville Dental, each smile is meticulously crafted using advanced surgical and restorative techniques tailored to the unique needs of our patients. Dr. Johnson Ozgur’s specialized expertise in complex dental rehabilitation transforms not only smiles but also significantly improves the quality of life, combining innovative digital dentistry with compassionate patient care.

This case perfectly exemplifies our dedication to precision and excellence. Our patient arrived facing significant challenges, including unstable anterior upper bridges and a severely compromised posterior bite, impacting her ability to chew and speak confidently. Her case required a comprehensive, surgical, and restorative approach utilizing state-of-the-art digital dentistry to fully rehabilitate function and aesthetics.

The patient’s journey began with extensive consultation and advanced digital documentation, incorporating sophisticated 2D and 3D design processes in collaboration with RDT Dental Lab. Utilizing CBCT imaging for precise surgical guidance, we strategically extracted compromised teeth and performed bone grafting with protective membranes, laying a solid foundation for future implant integration.

We provided the patient with a long-term fixed temporary bridge during the healing phase to ensure comfort, functionality, and aesthetics. After a successful healing period of over six months, digitally guided implant placement was executed meticulously, accompanied by additional bone grafting to optimize long-term stability.

Following complete implant osseointegration, MODJAW technology was employed to precisely document and analyze the patient's natural jaw movements. This innovative approach allowed for prosthodontic designs tailored to harmonize seamlessly with her physiological function, ensuring comfort and durability. Advanced digital design and manufacturing techniques delivered restorations with exceptional precision, enhancing aesthetics, bite function, and alignment.

The final implant-supported prostheses utilized a hybrid approach, expertly mimicking soft tissue lost due to prior bone resorption, significantly enhancing natural appearance and overall functionality. To further improve the longevity and ease of maintenance, screw-retained implant prostheses were chosen over cement-retained options, reducing peri-implantitis risks and facilitating future care.

To comprehensively finalize the rehabilitation, minimally invasive procedures with porcelain veneers, crowns, and onlays effectively corrected contact issues and crossbite, preserving natural tooth structure while significantly improving aesthetics and bite functionality.

The patient's experience and results were extraordinary. Thrilled with her outcome, she expressed profound satisfaction, highlighting the comprehensive care, emotional support, and exceptional attention she received throughout her journey. Her transformation restored not just dental functionality but greatly boosted her self-esteem and social confidence, solidifying a lasting and trusted relationship with our dental team.
